@God is my Savior . lookup uscis civil surgeon on their site. You enter your zip code and it gives you a list of all the known civil surgeons in your area. Call them and make an appointment. You should call more than one since they have different prices and procedures. It will likely cost you over $300 in any case so you can choose the cheapest one closest to you. Some will require you to print forms, others will provide it, and like the one I used, you can fill out details online. Either way make an appointment now. They will instruct you on what to bring with you to the appointment. There are certain tests only they can order so see them and they will advise.
