simple make a copy of the I-693 and search for a USCIS physician near you. They will complete the rest.

Google Uscis surgeon in your area, you can call like 3 or 4 to know their fees as it varies. Schedule an appointment with the office. once you get there, they will tell you the vaccines you will need to complete. call your primary care and inform them you want to get those vaccines. bear in mind that you will visit the surgeon office twice because you will have to go back to get your results which will take about 5days.

is the first time you are submitting medical? if is the second go back to the same doctor they have you on record
