-you can get medical records from the primary care physician. they always have the medical records -for church, if you are catholic, a baptismal, confirmation card will suffice. anyone in your church can also write -school records can be obtained from the school or even present school if available, like school results, school ID,cerificates from school -insurance records if the kid has insurance or yours if the kid is a dependent -your employment records to show financial coverage - census or tribal can be a sworn affidavit from the county showing where he/she is from. -govt record: will be birth certificate or sworn affidavit to show proof of birth. if you can provide most of the above that might work since you are not the biological parent. good luck!
