unpaid hospital debt, can it affect the process?
Can someone clear this doubt for me?

I dont know if it can affect it, but why dont you start paying? You can negotiate with the hospital and make a payment plan with a monthly payments you feel comfortable with, this way even if uscis see it they would know you're taking care of it
