@Mazza De i-131 On Part 1, fill out your basic information. Mark box 1.D. on part 2. Skip the rest of Part 2. Part 3, fill out part 1,2, 3.a. and check box 5 and skip the rest of Part 3. Skip Part 4, 5 and 6 as they do not apply to TPS. On part 7 section 1, check "more than one trip." Skip the rest. Skip Part 8. Fill out part 9 and you are done. Shouldn't take you more then 5 minutes to fill out.
