it usually is! USCIS can request as many RFEs as they see fit, however when you have your case approved and they’re only asking for medicals that’s typically a good sign and it’s exactly what happened to me. I got an RFE for medicals only then 3 weeks later I got my i485 approved, now I’m waiting for my card to be mailed.

they transferred new office for jurisdiction?

yes they transferred from Texas to NBC before send me my Medical RFE.
