medical examination
what vaccines do we need to have for medical examination? if anyone has a list i downloaded the 693 form and the few vaccines there are not age appropriate. if anyone has gone through this please help

usually MMR, dtap, and now covid and flu as well. if you have had vaccines you can have titers checked, if low then booster. most occupational medicine places are very familiar with the form

make sure you get the latest edit of 693. they were about to add Covid vaccine by Oct1st.

they charge you a lot and some surgeons don't do that

@abby Hatcher, i would appreciate you updating the vaccines you will get finally as i have to get my i 693 as well. i am thinking to get my blood test done from my health service provider and then get the vaccines which will be covered by the health service provider , and then go to the civil surgeon with vaccine record and blood test. What do you suggest ?? and how easy/difficult is to get civil surgeon appointment as the form needs his signature which cant be more than 30 days old i guess?? and does a civil surgeon gives us the missing vaccines if any, and how does he charge for that ??? than you
