Idk abour having to put the same address, but as far as the evidence for the I-765 form you need a passport copy, birth certificate and a translation if it’s not in English, copy of your visa + entry stamp (if you entered with a tourist/student visa), marriage certificate, I-94 form, passport style photo

@Princesse Nana you need a copy of the receipt of the 485, the receipt of the biometrics if you’ve gone already, a copy of the previous EAD if you have one, two passport pics, a copy of your ID, signed and complete form. for the address you can check the instruction form for 765 and based on your location you should get the address to mail it to. hope this helps.
