The only thing that you will have to do for your USCIS requested medical is to search on Google or the USCIS website for an immigration approved civil surgeon near you and setup an appointment for the medical. There is a little fee attached that is charged by the doctor or the medical facility. If you have taken other vaccines before and have documents to show, you must take them with you on the day of your appointment. upon the completion of the medical, the doctor will give you a package containing your results that you are never to open but give it to your lawyer if you have one to mail it to USCIS or if you don't have a lawyer, then yourself can mail the package including a copy of the communication sent to you by USCIS for the conduct of your medical.

You don't need anything. The civil surgeon will put their findings and comments on the form and sign it. That's all you need! What is a deficiency notice? I've never heard of that.

if you read carefully at the end of that deficiency letter it advises where to address your form and to include the copy of communication in order to speed up your processing. I hope that helps. talking from experience as I have just submitted one. thank you
