@A T 1. Birth certificates - This takes the longest. So start preparing right away. If you have spelling or first/last name differences compared to passport, prepare affidavit from yourself and parents/uncle/aunt. If birth certificates are not there, get Non Availability of Birth Certificate and 2 affidavits from parents/uncle/aunt. 2. Marriage certificate 3. All previous L1/H1 petition document copies and Visa copies. 4. Payslips for every month for entire duration of your employment abroad and in US. 5. Yearly assessment/review records from employer for entire duration of employment abroad and in US. 6. You need a passport photo each for each application. You can take this when it’s close to submitting application. 7. Make sure form versions are latest. So highly recommend filling out close to submission date and check this multiple times to make sure new revisions are not available. 8. I-693 Medical Records - Some of the vaccinations need series to be completed like Hepatitis B. So you can consult Civil Surgeon now itself and start those vaccinations. And 1 month before submitting I-485, consult Civil Surgeon for formal preparation of I-693 and have the closed envelopes ready. I can recall the above. Good luck. I hope date reaches your PD soon. @LC O Is birth certificate a mandatory requirement for AOS? Or any other proof of birth also works like a national ID card or overseas drivers license which has a DOB.
