we didn't attach any proof. just answered the questions in d form n signed it. then uploaded in d uscis online account

No proof needed, just fill out the information asked on the form have your spouse sign , I believe you also need the 1-9 arrival date that can be found on the link they provided if you don’t know the date and number.
