Two Passport Sized Photos (Petitioner and Beneficiary) 6. A Copy of Certificate of Naturalization (Petitioner) 7. A Copy of US Passport Biographical Page (Petitioner) 8. A copy of Birth Certificates (Petitioner and Beneficiary) 9. Copy of Unmarried Certificate (Beneficiary) 10. Letter of Intent to Merry (Petitioner and Beneficiary) 11. Proof of Meeting Within the Last 2 Years 12. Proof of Ongoing Relationship
