what notice do you need to put?

i actually faxed it again with cover page and printed my notice and added it too then thy approved it I have my Combo card now

I expedited twice ,the 2nd one was after 7working days I talked to a live agent for follow up n he said yes he can see that I expedite but thy havnt reviewed it I asked him to do another expedite
