Your birth certificate needs to include your full name, mothers name, fathers name and date&time of birth

you should try to get the consulate of your birth country to send you a new birth certificate. I had mine within a month

no not from the consulate in the US unfortunately, but you birth country which in your case is UAE. You must obtain the birth certificate from the issuing authority in your country of birth.

Supporting documents like affidavits from my parents are not enough?

I don't have personal experience so can't comment to the situation. Processing is purely dependent on how your officer chooses to proceed. My last name was spelled incorrectly and I had no issue for GC, citizenship and when I sponsored my parents. I see you have two possible choices 1. Submit your birth certificate as is and see if you get RFE. In the background continue working to get counsular birth certificate with full name 2. Get the counsular birth certificate but this might just delay things and add time

Not really familiar with counsular. But my best guess will be UAE consulate in US as you are born there.
