Birth certificate without name
USC filing for parent (IR-5). Parent has a birth certificate but the name was not included in the certificate. In this context, what is required ? Can an affidavit by elder relative suffice ?

no mention of the the "no name" clause in that link.. if no certificate is available, affidavit can be produced.. wondering if anyone had a personal experience in this regard
