There is a lot of conflicting information you get out there most people stating you have to notarize it. This is not the case, the instructions clearly state that you need a sworn or verified by third party statement. As long as you include “I swear, under penalty of perjury, that the foregoing is true and correct to the best of my knowledge.” at the bottom of the letter it counts as a sworn statement therefore you don’t need to notarize it.
