it’s a request for exemption from the I-864 form. If you are a US citizen then your children will become US citizen once they arrived in the US hence making them exempt from the original affidavit of support. The form is fairly simple and don’t need a lot of info as the I- 864.

@ Jaykym Hylton, thank you. I have view form and you are right, it only 5 pages and pretty basic compare to the I-864. However, there are some question I have: 1. If you are filling up for a Chile under 14yrs of age, who name goes to the Name of requester? 2. The mailing address is my US address but the form is not giving me the option to type in the Child's physical address if they are not presently living in the US. What do I do in this case. 3. If a parent is filling the form for a child under 14yrs, do I check any of the box in Part 3. regarding I can read English, the interpreter or preparer? sorry for all the questions but I just don't want to do something again that they will reject after almost two and a half months like they did with the I-864 I submitted in May.

hello Joe I have gone to the interview today embassy tell me to fill I 864w but this form is challenging me who is the name of requestor, on that social security is that for me or because my child is not yet in us she doesn’t have social security number and also the emailing address can I put mine , on physical address can I put address for me in us or the country where the child is @joe T thanks

but basically, the requestor is the name of the child for whom you are filling the I864 W.

@ Mary Fer, I864W is only for an examption for underage children. Is the person(s) you doing this form for underage? if not you should be using other forms.

If the person has no SSN, I put NA or just left blank

Thanks a lot Joe T. So, eventhough the SSN is stated as required, it is Ok to leaving it blank. Was it the same case with your approved form?

obviously when you are filing for a minor who is oversees, they wouldn't have a SSN. in my case, I left I had to put NA

I am having thesame issue, My I-864 was rejected on the note it says intended immigrant I- 864W is missing, my question is do nvc want me to replace the I-864 with I-864W?, I need help with this.

@AB BA, you simply have to download the I864W since you have minor child/childres complete and upload to NVC like you did with other documents. The time requires to do this and information on the form is pretty similar.
