@Hw orker - polio is mandatory. I didn't have records and my doctor wasn't ready to seal the envelope without the dose.
I went and got a polio shot and provided the proof. only then the doctor sealed the envelope.
id suggest you take the dose and keep the proof ready and if you get an RFE you'll be able to prepare the response faster.
incase you don't get an RFE - you only have an extra polio dose- which won't harm you !!!
