@K S usually any documentation from a medical facility should be sufficient. if you don't have such then you have to take the vaccine again or better take a titer, which is a blood work that shows you are immune to the disease the vaccine is for. Titer is very common and you can tell your PCP to get you one for most of the required vaccines.

@Dodomayana 007 I am thinking of doing the titer tests. I do have some vaccination records but not sure if they ll be accepted.

update: medical examiner ordered titer tests. I had to take varicella and flu vaccine. there is no titer for polio but they said it’s not needed according to your age.
