@Penelope charming it’s second dose for tdap. I believe the first medical exam was lost and they sent me exactly the same request for second one.

@uc j thanks! @yellow banana 🍌 I still work with the sponsor and have never changed employers. Form I-485 Supplement J is mainly used for two purposes: 1. To confirm a bona fide job offer from a new employer under the portability rules (AC21). 2. To confirm continuing job offer if USCIS requests it during adjudication (for example, after a long delay or status change like mine). So, • If you are still with the same sponsoring employer and your I-140 is approved, • And USCIS does not specifically request a Supplement J, then you don’t need to submit it proactively. However: If USCIS sends you an RFE or notice asking to confirm the job offer — even if it’s the same employer — then you must respond with Form I-485J signed by that same employer to reaffirm the offer.
