@aju It’s the I-693 form, has to be filled in by a civil surgeon and, depending on your type of your I-485, you have to either mail it in or take it to your interview. you don’t have to do anythigg by until you receive the ‘I-693 deficiency notice’ which will also state if you should mail it or take it to interview.
